So yeah, this has been bothering me for a bit, now. Pack Tactics provides a bonus to damage due to Swarm, but Uncontrolled Rage hits allies adjacent to a target for 50% damage. What this means, basically, is that friendlies contributing to swarm bonus are effectively committing suicide in the process - not just by being there, but also by increasing the Warrior's damage to themselves . This is an odd one, and I'm really sorry I don't have any visual record of it. I had Bacco the Beggar in a fight against a basic group of skeletons (the ones that spawn from the Night of Haunted Dreams random event quest). He has Reap 3, but had just recently become mobile again after a previous fight went badly for him, so his starting health was low. The "Pyromancer" trait in the Mage class tree states that it allows the user to cast Rain of Fire and Burn City, when in fact it does not provide access to either spell. This should either 1) be changed to remove this erroneous information or 2) be changed to provide the spells it refers to. Edit: Stupid smiley codes...
In the Assassin tree, there is an ability called Bleed. You can train it, but your champion never acquires the skill. It also does not seem to apply passively, so this is obviously broken and needs work.

So, the Assassin's Break ability... it doesn't work. 1) Ability does not apply damage (tooltip says it applies normal damage in the process). 2) Tooltip for enemy hit with ability does not show any reduction in armor. Tooltip for estimated damage does not show any increase. 3) Damage does not appear to be increasing (most pronounced against units with high base armor stat). Edit: Updated build number because bug still exists.

In current game, just hit the 200 fame mark and got a Champion. I was given options between a Warrior and a Mage. I chose the Mage. There should be a repeatable option similar to the Epic Stat Boost option presented in D&D 3.5, if nothing else. And yes, as parrotmath pointed out, though I did this by modding the core game, it is entirely feasible (if nearly banal) to do it within core game structure.
