I performed a cleanboot and attempted to run the .bat from StartStart10_11.zip. However an error appears as shown in the following screenshot. So I manually started Start11 from services.msc. After the service has started, I checked the start menu, sadly the results are the same.

@basj Thank You for your help @Neil I am not able to find the setting for experimental tile to see if it is on or off. Where is it located? The apps do launch ok (no icon after reboot). But curiously, a few of these apps (that are missing icons) will show the appropriate icon in the taskbar and some will not (just a blank square where it should be, with a line underneath indicating a open app).
After doing these steps, I'm sorry to report no change. If interested, I tried the Windows 10 style of Start11. Below are screen shots of the default Win10 menu and next screenshot shows Start11's Win10 menu. I'm still using Windows 10, but using Start11. I recently suffered a drive crash, and could not reload any recent backups. I managed to reload an image from 18 months ago, when I was still using Start10. The problem I am experiencing now is the Universal Apps Menu/List as shown in the screenshot. Some apps that are installed are missing its icon. These icons is also missing if the app appears on my frequently opened list. I have tried: uninstalled (using PurgeStart
