I'm having what sounds like the exact same problem. Whenever I apply a new icon package all of my application icons get set to the package's default. It looks like the Iconcache.db file gets wiped out. I made a copy of mine before I applied a icon package but was unable to get Windows to use it once I copied it back. I'm using Windows Vista Ultimate 64-bit. Icon Packager is pretty much worthless at this point unless I just want to use default icons. Any word on
