Start button *and* clock are gone ...

I'm not sure if this began with the 1.45 update, but my start button and clock are now gone. This is annoying, and productivity-crippling, to say the least.

I only have it installed/registered currently on one system, and am not getting activation notices like others are.

What's my next step? Roll back to 1.41?

I think I know the problem you are referring to - in part.  I don't know of any proper fixes and certainly don't know what caused mine to start happening.  Start8 button disappears but my clock doesn't actually disappear though it simply freezes and a couple of the icons are one on top of the other.  Of interest though, it seems to coincide with another problem.  The desktop items start to get a black surround when the mouse has been pointed at them and sometimes the background turns entirely black.  I only have 2 temporary fixes until I spot something that fixes it properly.  First I sign out then sign in again.  Fairly quick and restores the task bar icon issue to normal.  Right click on the desktop - personalise and select the background that was there before turning black.  it is then fine until it randomly decides to do it again.

I haven't updated to Windows 8.1 and wary of doing so until, as mentioned above, I spot some proper fixes.
