Ghost icons?

i'm running vista 64bit, and object dock works almost perfectly, the only problem is when i set it to display open windows there are a few programs that cause "ghost icons," they appear on the dock w/ no icon but they take up space and you cant get rid of them without restarting the dock.
here is how i usually come across them:
the first instance of firefox becomes a ghost icon on the bar. when the window is open, i can click on the icon to maximize/bring to focus the window but once i close it it still stays there. however, if i have one firefox window open and open another, it displays properly.
its weird because this doesn't happen when i launch firefox from the start menu, and i've remade the shortcut a few times (even one pointing to the .lnk shortcut) but same thing. it happens with opera too, but not internet explorer.
