Open and Save dialog icons stuck on default

IconPackager 3.10 (x86)

The Desktop, hard drive and folder icons remain the default in the Open/Save/Save As dialog windows, including the places bar. The My Documents, My Recent Documents, My Computer and My Network Places icons appear correctly. All icons appear changed correctly in Windows Explorer.
I've tried "Rebuild Icon Cache" and "Repair Icon Images," toggling Individual Drive mode, changing the icon for the .DESKLINK extension, the Quick Launch icons, but it changes nothing. If I enable Individual Drive mode and manually change all icons, that works, but I can't do that all the time because I switch IconPackages frequently and I'd still be stuck with the ugly Desktop icon.
I know the locations displayed in the places bar are handled in HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Policies\comdlg32\PlacesBar, but I don't know where the icon settings are stored.

If you haven't already, open IP, go to Tools/Repair Shell Icons, select "Repair shell icons for all processes" & click OK. You may have to reboot or restart Explorer, but that should do the job. I've found that updates to IP always reset the shell icons to the default & I have to do the repair after each update.
