Is that rounded square a Windows 11 thing? What causes this? Is there a solution to avoid this?
P.S.: I don't think it is an IP issue, so broken record "IP needs an update" ist not called for. 
Which graphics program (with version if possible) are you using, and on which OS?
Latest Photoshop 2023, Axialis IconWorkshop 6.91 and 
BUT: The original Icon doesn't look like that.

Also, check if you're using stroke or not.
