Can images saved in the .ICO format be larger then 255x255 pix?

You mean larger than 256x256 (just a guess)
As far as I know, 128x128 is the largest .ico format. (but I could be mistaken) Anything larger, is probably PNG.

Koasati: they can be up to 256x256. the Umicons set by manhattan (the real set, don´t know about the Icon Packager adaption) has them: http://www.deviantart.com/deviation/1869068/
larger is impossible as far as i know.
