Items disappear if you click on them but don't use them

I have had this issue a couple times, but last game I clicked on the ascian collar (can't remember the correct name) in a battle to see what the range was, then right clicked to cancel and the item disappeared from inventory without having actually used it. It has happened with a scroll of fireball as well.
