If you drag an icon and, while doing so, click on the Start button to close the menu, a ghost icon appears in the location where the dragged icon was last hovered before the menu closed. This ghost icon cannot be interacted with, even if you attempt to refresh the icons.
I have been able to reproduce this issue, although I'm not quite sure why you would want to do this. Restarting explorer.exe will bring the icon back to the menu, however it might not be back in the same exact position within the group.
