It appears to be very rare, only seen it twice (many games/planets & builds..). Playing as Yor both times and it was on their home world. No mods or race changes.
My planet was fairly well established, had the research project running, finished researching a mfg tech, auto upgrade put all the buildings in my planet's build Q.
Sounds OK, right? Problem is it put the new bldgs BELOW the research project 
. So my new improvements never started. All i had to do was cancel the RP and rstart it and everything was fine, but i lost a lot ot turns before i noticed the problem.
